Learning How Tools Improve Safety when Caring for the Acutely Ill Patient

This qualitative study explored the application and impact of risk scores, response protocols and communication tools in care of the acutely ill patient. Findings from observations andinterviews with patients, relatives and staff in two medical settings in two UK NHS Trusts showed that the tools were used as prompts, reference points and a resource.
Hospital Mortality Rates

The Trust reduced their mortality and infection rates following the implementation of care bundles. This has lead to a 15% reduction in mortality rates and a 60% reduction in infection rates for Clostridium Diffcile.
Improvement in the face of Adversity: Case Study

Year on year reduction in adverse event rate in a district general hospital This improvement project was undertake within Glan Clwyd Hospital (YGC), part of Betsi Cadwaladr University Health Board in North Wales. YGC is an acute hospital within a rural setting, serving a varied population and has a total of approximately 1000 beds.
Improving the use of Mortality Data

Mortality rates have become a national concern and are used increasingly as indicators of patient safety. Hospitals need assurance that clinicians are reviewing circumstances of deaths and responding to contributory factors. M&M meetings can provide this function but to be effective they need to have a structured process for identifying areas of concern and recording corrective actions.
The Paediatric early warning tool

Sheffeld Childrens Hospital has used a paediatric early warning tool since November 2005 to identify ward patients at risk of clinical deterioration. Throughout 2009 it was embedded into patient observation charts to improve its accessibility and use. Additionally a nurse outreach response team was piloted for 3 months.
